RULES AND REGULATIONS

Producer may qualify for only one vacation per calendar year.

Due to limited availability, only days qualified for will be awarded.

Award must be used by 12/31 of the eligible year.

Business must be issued and placed. Canceled and lapsed business will be subtracted from qualifying production.

Agent must be in good standing with DIS, Inc. and its carriers at the time of prize claim.

Agent must be currently contracted with DIS, Inc. from which qualifying production is derived.

All qualifying producers must be directly contracted under DIS, Inc.

No outstanding debt balance.

Contest credit dependent upon federal and state guidelines.

Prize qualification cannot be transferred.

Excess Premium does qualify for production credit.

Not redeemable for cash or other remuneration.

By invitation ONLY.

This is a personal vacation.

DIS, Inc. reserves the right to modify qualification requirements.

EXPENSES: The qualifier is responsible for all federal / state income tax incurred as a result of attending the trip. A 1099 in the amount of the “fair market value” of the trip will be issued. It is suggested that participants consult their tax advisor in regard to a tax treatment.

NO EXCEPTIONS TO CONTEST QUALIFICATIONS, GUIDELINES AND QUALIFYING PRODUCTION CALCULATIONS WILL BE CONSIDERED
